In line with the threat of the spread of COVID-19, please be advised that we are strictly implementing a screening process prior to your arrival date. Failure to answer this form may result in a cancellation of your booking or a delay in your check in. Kindly complete this health declaration form honestly. Giving false information is punishable in accordance with Philippine laws.

The establishment collects the following personal information relevant in the advancement of protocols and precautionary measures against COVID-19 Acute Respiratory Disease. The collected personal information will be kept/stored and accessed only by authorized personnel and will not be shared with any outside parties unless the disclosure is required by, or in compliance with applicable laws and regulations.

Hotel Policy

• Liability for valuables and effects of the Hotel's guest shall be governed by article 1998 of the Civil Code of the Philippines. An in-room safe is provided in all rooms. 

• No pets are allowed at The Sphere Serviced Residences. The Hotel reserves the right to charge guests a corresponding amount of violations.

• In compliance with government ordinance, the Hotel has a strict non-smoking policy. Smoking is prohibited anywhere other than in the Designated Smoking Area (DSA) located on the 2nd floor. The Hotel reserves the right to charge guests Php5,000.00  for violations and/or damages.

• Any reported damage to Hotel property or loss of items will be subject to a corresponding fee, which must be settled prior to check-out.
